APPLICATION AND REQUEST TO THE VERMONT DEPARTMENT OF TAXES FOR PAYMENT OF THE
HOMESTEAD TAX ADJUSTMENT TO TRANSFEROR(S)

The following information is included and attached to this Application and Request:
1. A copy of the completed property transfer tax (PTR) return that was filed with the town clerk is attached. The
PTR includes the transferors’ names and social security numbers;
2. Proof of payment of the property transfer tax (e.g. a copy of the check or receipt of payment by the town or
closing agent) is attached; and
3. The property’s school parcel account number (SPAN) is _______________________.
Please issue a check for the full amount of the property tax adjustment, made payable to the transferor(s), and mail
it to the following address prior to July 15, 2007:

Note to applicants:
If this request is mailed, send it to: Vermont Department of Taxes, P. O. Box 1645, Montpelier, VT 05601-1645. The
envelope should be marked “Homestead Closing”.

The transfer must occur after March 31, 2007 and before 4:30 pm on June 20, 2007- the date and time when all
applications for payment must be received by the Tax Department.
